Each bottle contains an information sheet for completion and is then placed back in the bottle and put in the door of the fridge. It also comes with two stickers, one for the front door of the person’s home and the second to be placed on the outside of their fridge. The stickers are recognised by the emergency services and enables them to quickly learn vital medical information and details of emergency contacts via the form.
